Treatment for Heavy Bleeding with Menstrual Periods

Endometrial ablation is a management for menorrhagia, or heavy bleeding with menses. It involves altering the endometrium so it doesn't produce as much blood and fluid for the cycle. This is a procedure that can be done in the office or under sedation in the operating room. After the procedure, 95% of women have an improvement, and 30 to 50% of women have no menstrual bleeding thereafter. We can do either ThermaChoice or NovaSure in the office.

Procedures for Stress Urinary Incontinence

Urinary incontinence is very common - it increases through life, and most women are hesitant to seek management. The midurethral sling is the gold standard for stress urinary incontinence. Technology and mesh is advancing on a daily basis, so there are many options for sling placement. Most of the slings are done on an outpatient basis in the surgery center, but a few can be done in the office.

Periurethral Bulking

This procedure involves no incisions - it involves injecting a material at the junction of the bladder and urethra to provide support at the urethra for continence. This causes improvement in 75% of women. It can be done in the operating room and sometimes in the office.

Pelvic Floor Prolapse

Weakness or hernias in the pelvic floor can cause problems with the bladder and the bowel, as well as pressure and discomfort. Pelvic floor repair can correct these problems. It is important to evaluate for the many types of prolapse and correct each based on the anatomy and the needs of the individual. There are conservative methods to improve the pelvic floor and also many different surgical methods to repair the defects. Advancing technology gives us many methods to tailor to the specific needs of the person.

Permanent Surgical Sterilization in the office

Permanent surgical sterilization can now be done in the office. Essure is an office procedure that blocks the Fallopian tubes permanently, also known as tubal ligation. This is done with no incisions, using hysteroscopy, or a camera in the uterus. A small device is placed in each tube that blocks it, causing sterilization. There is minimal recovery. A dye test needs to be performed in three months to confirm that the tubes are blocked, but then the procedure is considered permanent and secure. Tubal ligation can decrease women's incidence of ovarian cancer.
